Requesting records for court purposes
A patient may request their records and a caregiver may request their demographic information maintained by the Medical Marijuana Registry.
To request records, email medical.marijuana@state.co.us with the following information:

A copy of the patient or caregiver's valid Colorado ID.

A completed request form.

Patients comple the Release of patient information form

Caregivers complete the Release of caregiver demographic information form

We are not able to release any records without the individual's consent. An attorney may submit the required information on behalf of a patient or caregiver.
Pursuant to our approved Records Retention and Disposition schedule, we only maintain records for the current active registration and the previous year's registration. Application and other documentation older than two years are not available.
Subpoena and appearance fees
The Medical Marijuana Registry has the authority to establish fees for subpoenas and time spent traveling and appearing in court pursuant to C.R.S. § 25-1.5-106(15)(a) and 5 CCR 1006-2 Regulation 13.
Remote testimony

Two hours or less of appearance (by phone or video call): no cost.

Two to four hours of appearance (by phone or video call): $200.

Four or more hours of appearance (by phone or video call) will result in a total fee based on the following formula:
| $200 | for the first four hours of appearance and on-call time |
| $ | + Actual hourly rate of the witness employee x subsequent hours |
| $ | Total subpoena fee |
In-person testimony

Four hours or less of appearance, on-call and travel time to court: $200.

Four or more hours of appearance, on-call and travel time to court will result in a total fee based on the following formula:
| $200 | for the first four hours of appearance, on-call and travel time |
| $ | + Actual hourly rate of the witness employee x subsequent hours |
| $ | + Per diem x number of days |
| $ | + Mileage (roundtrip to and from court) x number of days |
| $ | + Lodging x number of nights (if necessary) |
| $ | Total subpoena fee |
